Objective

Students will become familiar with the theoretical premises of case management and the
phases of its process. Students will recognize case management methods and
individualized needs and services of the elderly. Students will know hot to evaluate the clients’ individualized service
paths and point the necessary support and guidance. Students will explain multi-professional
cooperation and networking.

Content

Project work. Theoretical justification of case management, phases and work orientation of
the case management process. Multi-professional networking. Assessment of elderly care
service needs and service planning and arranging.

Assessment criteria, satisfactory (1)

The student is able to:
- apply professional knowledge in elderly care and gerontology appropriately
- use concepts and information in elderly care and gerontology systematically
- argue a course of action on the basis of research information and findings
- find information and argue the use of his sources

Assessment criteria, good (3)

The student is able to:
- use concepts and knowledge in elderly care and gerontology fluently and comprehensively
- assess and use information sources and research information with a critical eye

Assessment criteria, excellent (5)

The student is able to:
- display a good understanding of professional information in elderly care and gerontology
- use theoretical and professional concepts and knowledge like an expert and combine them
into larger entities
- create and communicate new information
