Objective

At the work placement (5 credits), students are able to conduct distance and near refractions and assess the binocular status. The eye examination includes refraction, and assessments of near vision and binocular vision. The students are able to evaluate vision and visual problems at different viewing distances utilising various methods of measurements and assessments.
Students are able to recognize needs for referral and are capable of customer-oriented communication on vision, visual problems and eye care.
After completing the study unit, students are able to suggest orthoptic treatments, in which the test results and visual needs are considered. as well as their link to the symptoms. They are able to justify the need for treatments and can link them to the symptoms. They know how to plan and carry out orthoptic treatments.
The students acquire skills in the second domestic language (Swedish) that are necessary in their professional field, as required by national legislation (352/2003 8§). The students acquire verbal and written language and communication skills that are at least at the B1 level as defined in the Common European Framework of Reference for Languages. They are able to seek optometric information from various sources and can convey both verbal and written information.

Content

The Golden Standard for Eye Examination, assessing and solving problems relating to binocularity and accommodation. Determination of a prism correction according to the Sheard and Percival criteria, utilising spherical powers with high ACA values, graphical analysis, high refractive error, examining children, anisometropia, amblyopia, ARC, determining the need for orthoptic treatment and implementing it. Customer service and communication in Swedish.

Previous courses in eye examination (sx00bp25 and sx00bp26) shall be passed before (by 6th January 2019) participating in this course
Lessons in Swedish : 301. 4.2. 11.2. and 26.2.
Exams in Swedish: 3.3.
Work placement: 3 weeks. Student have to do:
• 10 eye examinations that consist of preliminary tests (H-test, cover test, accommodation amplitudi, NPC, pupil reactions), retinoscopy and refraction, near vision assessment and binocular vision assessment and prescribing for spectacles.
• Moodle web-based assignment on one of binocular vision case
• Moodle web-based exam on one of dispensing case; spectacle lenses for a presbyopic (lens type, vision depth in various areas of lenses)
• Observe 2 eye examination (optometrist/ophthalmologist) and reflect that to the Gold Standard and ECOO recommendation.
Detailed content of the course (not include Swedish):
*Duane classification on the heterophoria and AC/A: Divergence excess, Basic exodeviation, Convergence insufficiency, Divergense weakness, Basic esodeviation, Convergence excess.
*Anisometropia and aniseikonia: influence to the binocularity, static and dynamic aniseikonia, test and measurement of aniseikonia, treatment and management with spectacle.
*Amblyopia: Definition and risk factors: hyperopia, anisometropia/antimetropia, stabismus, role of the emmetropization, Treatment: occlusion therapy, penalization, treatment with drugs
*ARC and NRC: theoretical background, examination of the retinal correspondence: Bagolin and covertest
*Concomitant and comitant deviations: specific symptoms and double vision, intermittent and constant deviation
*Suppression: evaluation
*Binocular refraction with fogging method.
*H-test with pen light: evaluation of the binocular vision field.
*Typical asthenopic symptoms in deviations and accommodation problems: anamnesis
*Cases: treatment and management of the binocular vision problem patient including the classification, analyze of the AC/A, Sheard and Persival criterions, horisontal and vertical prism, Fresnell, spherical power lenses and vision therapy, problem oriented response (management/ spectacles) to anamnesis, descriptive and ICD 10 patient recording.
*Vision therapy: theoretical background, motor and sensory activation, plan and management to alleviate the symptoms, physiological diplopia as a suppressio control
*Vision therapy in the accommodation problems: exercises (Near-far jump, Brock-string, push-up, flipper)
*Orthoptic exercises for the convergence insufficiency and divergence weakness (anti-supression, Brock-string, stereograms cards, two pencil)
Vision Therapy: plan and management process: Symptoms and identification, management plan, exercises (independent and office), patient advising, control visits including control of spasm and final evaluation, further management plan. Vision therapy process with (fellow student) patient including examination, choosing the exercises and control visit.
*Grafical analysis
**Special Cases in Eye Examinations:
*Pediatric optometry (primary school): Vision examination and patient based communication. Development of the child and how it affects to the communication and concentration. Examination: Lea symbols, Teller cards, forced choise tests, retinoscopy, cover test, Hirschberg test, patient history taking with parents, refraction with trial frame, H-test with pen light, recognizing the risk of amblyopia, visual acuity in childhood.
*Elderly patient: Vision examination and patient based communication. Physiological changes in vision, elderly patient´s communication and services. Eye examination: accommodation, pupil size, depth of focus, Amsler-chart, IOP.
*Client service in eye examination: how to take into account patient with different cultures, religious, backgrounds, disability (e.g. hearing, wheelchair) in eye examination and client service.
