Objective

Students will become familiar with the theoretical premises of case management and the phases of its process, and be able to apply case management methods to the integration and organization of the individualized needs and services of the elderly. In the different phases of service planning and implementation, they will evaluate the clients’ individualized service paths and the necessary support and guidance. Students will draw on multi-professional cooperation and networking. They will apply the principles of dialogic customer work to empower the resources of the elderly.

Content

Project work. Theoretical justification of case management, phases and work orientation of the case management process. Multi-professional networking. Assessment of elderly care service needs and service planning and arranging. Drawing up an individualised case management plan.
Work placement (7 credits) in various elderly care environments, e.g. nursing homes, assisted living homes, service and rehabilitation centres, homecare, family care, memory wards, social and welfare work, polyclinics, hospital wards, senior services, organisations, projects and directing volunteer work. Self-evaluation of competence and professional growth.
